2016 Skin Form
If a wrestler had a recent skin infection still with visible marks or has any questionable marks on their skin, they must be cleared by a physician and have a valid skin form during weigh-ins and on hand to show in case of questing prior to any wrestling match.

This form is valid for Area Qualifiers, Area Championships and State Championships series and must be completed and signed by a physician.
